​Withdrawal Form

Transfering?

Thank you for sharing with us your most precious treasure, your child. We are sad to see them leave our school but we understand your desire for them to continue their academic studies in another school.

In preparation for their transition to another school, DISK requests that the Student Withdrawal Form​
is completed and returned to school at least 5 working days before departure.
Please be aware that once you are in DISK and part of the Doshisha family, this does not automatically grant you transfer to our other schools as each has unique entrance requirements.

For example:
Doshisha International Junior/Senior High School has these requirements: ​www.intnl.doshisha.ac.jp/admission/qualification/

Specifically: only overseas returning students who have been in the country less than the number of years overseas may apply
​We wish your child success in his/her academic pursuits. Please let us know if we can help support them in anyway in making a smooth transition to the next school.

Transcript and Document Request​

If you are needing to request DISK official school documents please download and complete the appropriate document below and return it to school as soon as possible. Once we receive the completed form, it can take 5 to 10 days for the documents to be prepared and mailed. Please note there is a charge for mailing outlined in the document.

Documents containing the school Hanko can only be mailed directly to other institutes. They can not be sent to parents.
