If you are needing to request official DISK school documents please download and complete the appropriate document below and return it to us as soon as possible. Once we receive your completed form, it can take 5 to 10 business days for the documents to be prepared and sent.

- If you are applying for documents to be sent to a foreign country (outside of Japan) there is a delivery fee. This varies depending on weight and destination.
- Documents containing the school Hanko can only be mailed directly to other institutes. They can not be sent to parents or students.
